（1） Entrust
1. Identification materials of the applicant;

4. The text of the power of attorney must be determined. It is recommended to use the general text of the place where the power of attorney is used. （2） Statement
1. Identification materials of the applicant;
2. Information related to the declaration;

（3） Birth / birth certificate
Birth notarization can be divided into birth medical certificate notarization and birth fact notarization.
Notarization of birth medical certificate: the birth medical certificate is attached to the notarial certificate without pasting the applicant's photo. （5） Fingerprint pattern
(1) Identification documents of the applicant and present in person;
(2) Fingerprint card (please follow the tips on the fingerprint card.
